The Health Homes will operate under the Psychiatric Rehabilitation Program. Health Homes will assist individuals with serious and persistent mental illness in improving their overall wellness. It is a strategy to help individuals manage their health better and live in the community longer. We will take a person centered approach to addressing the individuals behavioral, somatic, and social needs.
